We accept all Canadian Provincial Insurance plans :

  • Out of Country please come into the clinic 

  • Blue Cross Refugee Program please bring up to date documentation.

  • OHIP Please download and bring forms.

There are additional fees for completion of forms for use outside of the patient's medical record, including:

  • Return to work/school notes

  • Missed Appointments - without 24hrs notice

  • Disability Forms - come into clinic to set appointment for forms

  • Transfer of Medical Records to a Third party

  • Absentee Notes

  • Referral for Physio/Massage or Orthotics

  • Disability Tax Credit Forms

  • School/Camp Fit for Work Forms

  • MTO Physical 

  • APS Completion - Short/Long Term Disability Forms

  • Liquid Nitrogen Treat Per Lesion 

  • No OHIP Card/ Out of Country Patient  

  • Invalid or Damaged Health Cards

  • Travel Immunizations 

  • Transfer of Medical Records 

  • Faxing & Photocopying
